Audiological Consultation with BOCES FM Equipment
This service is for students for whom an FM system is needed (to include but not limited to hearing loss and Central Auditory Processing Disorder) and who currently are not receiving services from an Erie 1 BOCES Teacher of the Deaf-Hard of Hearing.
Ideal for students utilizing a BOCES owned FM equipment due to Central Auditory Processing Disorder (CAPD) or hearing loss and not currently receiving services from an Erie 1 BOCES Teacher of the Deaf-Hard of Hearing.This service includes monitoring/troubleshooting of district owned FM equipment as needed. Accessory replacement is provided. District is responsible for the purchase and repair of the FM equipment along with replacement/servicing when necessary. One audiological/FM fitting/FM evaluation or CAP evaluation (if appropriate) included, pending district obtaining parental consent to evaluate.
District Responsibilities
- Complete BOCES referral form
- Share the IEP
- Provide relevant educational information
- Notification of CSE/Annual Review Meetings
- Notification of Amendment Meeting
- Send re-evaluation requests
BOCES Responsibilities
- Provide services as per the IEP
- Provide monthly monitoring
- Provide in service training for staff
- Perform formal evaluations
- Participate in CSE/Annual Review Meetings
- Collect data for progress monitoring
