Extended School Year Program (Summer) - LPN

The service must be specified on the IEP. The LPN/individual care aide would provide medical assistance/support based on the student's Health Care Plan. The LPN could also assist students in activities of daily living skills. Sharing of LPN aide is encouraged.

Note: Students requiring this service will not be admitted into program unless the position is fully staffed.

974.000 - Enrollment in Extended School Year Program

District Resonsibilities:

1. Must indicate need for LPN on the registration form
2. IEP must indicate LPN for Extended School Year
3. Provide updated medical information/records
4. Provide medical equipment (if applicable)

BOCES Responsibilities:

1. Provide supervision
2. Keep daily nursing notes
3. Perform formal evaluation of LPN
4. Develop and monitor Health Care Plan
5. Develop and maintain emergency protocols
6. Review scripts
7. Work collaboratively with the summer program's school nurse
